You have the right to ask for a copy of the personal information we hold about you. This is known as a Subject Access Request (SAR), and it's your right under UK data protection law.
Making a request
You don't have to use any particular form or wording — you're welcome to contact us however suits you. However, completing the short form below helps us deal with your request quickly and get your information to you without delay, as it captures everything we need in one place.
What happens next
Once we've received your request and confirmed your identity, we'll gather the information you've asked for and respond within one calendar month. If your request is particularly complex, we may need a little longer — up to two further months — and we'll let you know within the first month if that's the case. There is no charge for making a request.
Requesting on someone else's behalf
If you're asking for information on behalf of someone else — for example as a family member, advocate, solicitor or case manager — we'll need to see evidence of your authority to act for them before we can release anything. The form will guide you through this.
Your information, kept safe
We take care to protect everyone's privacy. Where our records mention other people, we may need to remove or redact their information before sending your file to you. We'll always send your information securely.
